Management of peri-implant mucosa with the buccal pedicle flap

Dr. Giorgio Tabanella

The Buccal Pedicle Flap, developed by Giorgio Tabanella, represents an advanced surgical technique for the management of peri-implant soft tissue deficiencies, particularly in the aesthetic zone. This approach is designed to enhance buccal tissue thickness and improve soft tissue architecture through the use of a vascularized pedicle flap.

Unlike free grafts, the Buccal Pedicle Flap maintains its original blood supply, ensuring superior tissue survival, integration, and stability. The flap is mobilized from the buccal aspect and repositioned to augment areas of volume deficiency, allowing for a biologically favorable increase in soft tissue thickness.

This technique is particularly indicated in cases of thin biotype, soft tissue collapse, or peri-implant deformities resulting from improper implant positioning or previous periodontal disease. It enables the reconstruction of gingival convexity and supports papillary fill, both critical factors for achieving optimal aesthetic outcomes.

The Buccal Pedicle Flap reflects a shift toward biologically driven peri-implant therapy, where long-term success is defined not only by implant survival, but by the restoration of stable, biomimetic soft tissue architecture.
