Managing complications: step by step clinical procedures

Dr. Giorgio Tabanella

Complications in dental medicine remain a significant clinical concern, impacting both treatment outcomes and patient quality of life. These adverse events may arise during or after surgical, restorative, endodontic, or implant procedures, and can range from minor, self-limiting conditions to severe, irreversible damage. Common complications include infection, bleeding, nerve injury, implant failure, peri-implant disease, and aesthetic deficiencies.

The etiology of such complications is multifactorial, involving patient-related factors (systemic health, oral hygiene, anatomical limitations), clinician-related variables (experience, technique, planning), and material or device-related issues. In implant dentistry, for instance, improper three-dimensional positioning and inadequate soft tissue management are major contributors to both biological and mechanical complications.

Early recognition and accurate diagnosis are essential to prevent progression and to ensure timely intervention. Advances in digital dentistry, including three-dimensional imaging and intraoral scanning, have significantly improved treatment planning and risk assessment. Moreover, minimally invasive techniques and biologically guided approaches have enhanced the predictability of outcomes while reducing complication rates.

A comprehensive, patient-centered strategy that integrates careful diagnosis, risk evaluation, and evidence-based protocols is fundamental. Emphasis should be placed on prevention, including proper case selection, meticulous surgical execution, and long-term maintenance programs. Ultimately, the successful management of complications in dental medicine requires a multidisciplinary approach aimed at restoring both function and aesthetics while preserving long-term tissue stability.
